Out-of-Network

If my insurance isn't listed, can I get reimbursed by my insurance "out of network"?

  • Most likely, yes! Most insurance companies reimburse occupational therapy services. You can submit your provided receipt directly to your insurance in combination with a doctor's prescription for occupational therapy outpatient services. Make sure you call them and use our Insurance Benefit Worksheet to ensure that they will reimburse you.

  • Prior to your first scheduled appointment, call your insurance company to completely understand your occupational therapy benefits. In the forms section below, you will find an insurance benefit worksheet to help you ask the insurance company the right questions about your occupational therapy benefits. If you need any further assistance, please email info@integratedfeminine.com

  • At the time of service and payment, you will receive a written statement/receipt that you can then submit to your insurance company for reimbursement to you. The amount of reimbursement you receive will vary according to the terms of your insurance policy and out of network benefits coverage. We cannot guarantee or estimate what reimbursement your insurance will allow.

  • Once you've clarified your "out of network" insurance benefits, ask your doctor for a referral. You can provide this referral form to your doctor for them to sign. You will send this signed form and receipt from us to your insurance company for possible reimbursement.

  • This Insurance Benefit Worksheet will guide you through talking with your insurance company regarding understanding how out-of-network benefits apply:

  • Billing Codes Your Insurance May Ask You to Provide: When seeking reimbursement for outpatient occupational therapy, this worksheet list the typical codes used for billing. On the first visit, you will be charged an evaluation (and possibly 1-2 other charges if treatment is started).  Your insurance customer service representative may ask for the codes used for occupational therapy and you can find all the codes in the attached worksheet.
